The Initial Memory Coach Consultation Service is a new patient appointment, which can be scheduled as an in-person meeting, videoconference, or phone call.  The duration is customized to each individual patient's unique situation, but is typically about one hour.  Family members and loved ones are not only invited to attend, but are encouraged.

*  Complete medical history is documented, all the way back to childhood

*  History and course of memory and cognitive complaints is carefully recorded 

*  Formal memory care assessment is administered (typically either MoCA or Clinical Dementia Rating)

*  Family history is covered

*  Detailed medication reconciliation and assessment is documented

*  Lifestyle details are discussed and initial dementia risk factor assessment is developed

*  Initial recommendations and next steps are suggested
